Please note that registration is now closed for the Spring '08 Recreational Season. All age groups are at maximum capacity. Please check back in early September for the Fall season.

Players:

Please complete the registration form below. You will then be prompted to pay with a credit card via Pay Pal. Players will not be registered and will not be eligible to play until we have received confirmation of your payment. Per-Season Fees:

1.      Recreational League: $65.00 per player, $130.00 maximum per family.

2.      Mini-Kickers: $45.00.

3.      Competitive / Travel League: $175 per player. $350 per family maximum.

The FOVS Program may use photographs in which my child appears. I, the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of the MYSA, the USYSA, its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer and in consideration for the MYSA/USYSA accepting the registrant for its soccer programs and activities (the "Programs"), I hereby release, discharge and/or otherwise indemnify the MYSA/USYSA., its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the Programs, against any claim by or on the behalf of the registrant as a result of the registrant's participation in the Programs and/or being transported to or from the same, which transportation I hereby authorize.
